Health certificate questionnaire Domestic & International Health Certificates Regulations for a pet traveling outside of the continental US is governed by the nation the pet is traveling to. Those regulations and requirements may change at any time and without notice. The requirements and forms may be complex and may not be readily available to us. Los Altos Veterinary Clinic takes our guidance from the United State Department of Agriculture Animal and Plant Health Inspection Service (USDA, APHIS). However, ultimately it is the pet owners responsibility to insure all requirements have been completed correctly and in a timely manner. Please refer to the USDA website for travel information. Errors can happen while we help facilitate your pets Health Certificate and we DO NOT guarantee you will not experience delays or refusal of entry; however, a Pet Transporter will extend guarantees. We highly recommend that you use a pet transporter. Please acknowledge review of policies: I understand the regulations for travel for my pet are regulated by the States, country or countries my pet may visit. The costs associated with any additional treatment and/or quarantine, and/or the cost associated with the refusal for the pet to enter the State or country of destination in solely my responsibility as the owner. Timing of all vaccinations, testing, medications and appointments are the owner’s responsibility and if errors are made owners will incur additional costs to repeat services needed. We make no claim of warranty, suitability or acceptability for international health certificates beyond our endorsement on the forms provided by the owner and/or USDA-AAPHIS office or USDA-AAPHIS website. We do not warranty in any way that the pet will not be quarantined. It is strongly recommended that you contact a professional transport company to assist with the export requirements for your pet's travels. To prepare for your upcoming health certificate appointment, please provide the following information: Date of Flight Date of Arrival Name of person traveling with the pet Your current address Destination address Phone number you can be reached at during your stay Your pet will be traveling in Cabin Cargo Per the USDA, new travel regulations require a transit certificate for pets who have a layover in another country. For example, a pet flying from SFO to Paris with a layover in the UK will now be required to have BOTH a UK and Paris certificate filled out and endorsed. There is a charge for both certificates to be endorsed. Does your flight have a layover? Yes No If you answered yes, what country is your layover in?
